Policy to Protect Personal Information

In an aim to provide reliable healthcare, we are making efforts daily so that patients can receive quality healthcare. We believe it is very important to protect and manage personal information of patients. For this reason, we have established the following Policy to Protect Personal Information and will strive to execute it properly.
1.The collection of personal information
We will collect the personal information of patients to the extent necessary for the treatment, nurse care and medical care of patients. If personal information is to be used for any other purposes, we will notify the patient of the purpose and use the information with the consent of the patient.
2. The use and provision of personal information
We will not use the personal information of patients beyond the bounds of our intended purposes of use (refer to the attached 'Purposes of using personal information in Hannan Hospital') with the following exceptions.

- When consent is obtained from the patient.

- When personal information is used for the purpose of identifying an individual (Note 1).

- When personal information is requested by law.

We will not provide the personal information of patients to a third party (Note 2) without the consent of patients apart from when it is required by law.
3. Appropriate management of personal information
We will strive to maintain the personal information of patients in an accurate and up-to-date condition and prevent its leakage, loss, destruction, alteration and illegal access.
4. Verification and correction of personal information
In the event our Hospital is requested by patients to disclose their personal information, we will verify the content promptly and respond to the request in accordance with our 'Guidelines for providing patient information'. In the event that correction is requested due to inaccuracy of information, we will investigate the matter and respond appropriately.
5. Enquiries
Enquiries relating to our Policy to Protect Personal Information or personal information of patients are dealt by the 'Personal Information Protection Desk', General Affairs Section, Management Department on extension number 131-104.
6. Compliance with laws and regulations on personal information
We comply with the laws of Japan on personal information and other regulations.

Note 1
Information such as names of individuals is not merely made anonymous by deletion, but is retained in a form that is unidentifiable.

Note 2
A third party refers to those other than the individual of which personal information is collected and the recipient and refers to a group or an individual which does not fall under the intended purposes of use or which has not obtained the consent to use personal information from the individual of which personal information is collected.
