  • You have the right to have your personal information protected, which is obtained in the course of treatment.
  • You have the responsibility to provide information about your health as accurately as possible to healthcare professionals.
  • You have the responsibility to be careful not to interfere with the treatment of other patients so that all patients can receive the appropriate healthcare.
